NE HIE Integration Set to Better Population Health Management

A Nebraska HIE has partnered with a health IT vendor to improve patient care for high-risk patients through a population health management platform.

CyncHealth, a non-profit health information exchange (HIE) in Nebraska, announced a partnership with HealthEC, a population health management organization, to enhance patient care and clinical collaboration.

HealthEC population health platform portal will integrate CyncHealth’s healthcare payer, provider, and HIE data with clinical EMR data to assist providers in detecting high-risk patients that could benefit from greater connectivity.

Additionally, the population health platform will help manage provider collaboration during the delivery of care to ensure patient experience improvements, population health enhancement, and per capita cost of care reductions.

“The expertise provided by HealthEC has already proved to be an invaluable asset for CyncHealth,” Jaime Bland, president and CEO of CyncHealth, said in a statement.

 “It’s exciting to have a set of strategic methods and real-world tools to overcome the challenges that providers, payers, and others in our state experience every day. I believe that HealthEC’s near real-time data analytics and risk stratification capabilities will help us greatly improve lives for the up to 2 million residents that walk through our doors,” Bland added.

The partnership will also permit CyncHealth access to a patient portal for high-risk populations, which encourages individual engagement, benchmarked dashboards for the health department and its provider community, and analytics aimed at improving care coordination across the continuum.

CyncHealth will work on patient access, primary care, and behavioral health initiatives to offer over 2 million patients the right care at the right time, the press release stated.

Data sharing for population health management has played a vital role in enhancing patient care and is a popular strategy among HEIs.

In 2021, HEALTHeLINK, the HIE for Western New York, worked on 83 projects to enhance data sharing for population health management.

“Understanding the unique role HIE has within our local healthcare structure, HEALTHeLINK is working to deliver data needed for value-based care, social determinants of health, and population health initiatives,”  Anthony J. Billittier IV, MD, FACEP, chair of HEALTHeLINK Board of Directors, said in the HEALTHeLINK 2021 report.

According to a recent report, the HIE provided over 950,000 clinical results to providers, such as lab tests and radiology reports and images.

In addition, providers used patient record lookup more than 6.5 million times to gain instant access to patients’ EHR data.
